Regaining stability after an ankle fracture – combined aquatic and land-based rehabilitation

A young male patient visited us following surgery for an ankle fracture, based on the recommendation of his trauma surgeon.

He noticed that after the operation his range of motion had only partially recovered. As the surgical wound had already healed, we were able to prescribe a combined treatment programme.

The medical metal implant used to fix the fractured bones can itself restrict movement. Following the injury and surgery, a haematoma (blood collection) often develops, causing tension in the tissues and resulting pain. Due to the period of restricted movement prescribed early on, the joint capsule and ligaments may stiffen, further limiting mobility. As a result, the patient’s ankle movement may become restricted, making walking more difficult. In such cases the beneficial effects of thermal water temperature and hydrostatic pressure are particularly important, as they positively influence venous and lymphatic circulation as well as the function of the joint structures. In addition to the well-known advantages of hydrotherapy, underwater physiotherapy offers further significant benefits. The ankle is essential for gait stability, and its injury often leads to a sense of insecurity and balance problems. Water naturally surrounds and supports the body, providing a sense of safety during movement. Buoyancy reduces load on the joints, making the exercises easier and more comfortable to perform. At the same time, the resistance of water acts as a natural form of muscle strengthening, helping the muscles rebuild and restoring movement functions more quickly.

To stimulate lymphatic circulation, the patient received medical massage, and low-level laser therapy also supported the accelerated healing process. Naturally, the physiotherapist-led ankle rehabilitation exercises were an integral part of the programme. Thanks to this combination of treatments, our patient completed the course with a 40% improvement in ankle function.
